**Please send your money and order form in an envelope to your child’s teacher by September 21!. Checks MUST be written to Hazelwood PTO!!

The first 300 paid students will be admitted to the event!!

Your child will need to be picked up no later than 5:15!!! You will pick your child up in the carline. You MUST have your child’s car tag!
