HOW DO I CHECK THEM OUT?
● Print out this form and return it to Mrs. Clarke with the item circled that
you would like to check out.
● Email Mrs. Clarke to request an item – cclarke@haywood.k12.nc.us
● If something is already checked out, Mrs.
Clarke will put it on hold for you.
